On your first visit to our office you will be welcomed as a member of our family and our receptionist will greet you with a warm smile. You will then be asked to fill out our required forms which will help us gain some information about yourself and your condition.

Do I need to bring anything with me on my first visit? When you arrive for your first office visit, you will be greeted by our front office personnel, who will obtain your information such as a picture ID and your insurance coverage card.

If you have been involved in an automobile accident, please bring a copy of the accident report, and a copy of your own auto insurance card. We will then contact your Claims Adjuster and help you to file your claim. If you were hurt at work, you need to have already filed a report with your employer. We will speak with them to help file your claim with their Workers Compensation Carrier.
