On your first visit to our office you will be welcomed as a member of our family and our receptionist will greet you with a warm smile. You will then be asked to fill out our required forms which will help us gain some information about yourself and your condition. If you wish you may submit these forms securely online to save time. Submit your Secure Health History Forms online Do I need to bring anything with me on my first visit? When you arrive for your first office visit, you will be greeted by our front office personnel, who will obtain your information such as a picture ID and your insurance coverage card.
My insurance company is not on your list. I don't have any health insurance right now, can I still see you? Yes, individual and family payment plans are available.
If you have been involved in an automobile accident, please bring a copy of the accident report, and a copy of your own auto insurance card. We will then contact your Claims Adjuster and help you to file your claim. If you were hurt at work, you need to have already filed a report with your employer. We will speak with them to help file your claim with their Workers Compensation Carrier.
Do you need directions to our office? The length of your visit may range from 20 to 30 minutes, depending upon your condition. We recommend that you wear comfortable clothing in order to move about freely during the evaluation and/or treatment process.
