There will be charge which includes your required co-insurance and a convenience fee at the time your appointment is made.

• We require a credit card on file in order to reserve your appointment.
• Your credit card will NOT be charged until the day of your appointment.
• There is a non-refundable  $45.00 charge if 24 hour cancellation notice is not given.
• There is NO charge for rescheduling your appointment 24 hours in advance.
• Please follow your Recommended Care Plan.
